Information you submit in a form or email: typically your name, email address, phone number, zip code and a short description of what you need.
Basic usage data collected automatically: pages viewed, device and browser type, and approximate location from your IP address.
Advertising cookies: we use the Google tag from Google Ads. It sets cookies that tell us when someone who clicked one of our ads goes on to submit a request, so we can measure which ads work. The answers you type into our form are not sent to Google.

Our request form asks about the kind of concern you want help with and is processed by Jotform. We use that answer only to match you with a provider and share it only with the provider we match you with.
Please do not send claim numbers, diagnoses from your records or other medical details beyond what the form asks. Your provider will collect what they need through their own secure process.
Your choices
You can ask us to see, correct or delete the information you sent us by emailing info@bestdoldoctors.net. You can opt out of text messages at any time by replying STOP.
You can block or delete cookies in your browser settings, and turn off personalized ads from Google at adssettings.google.com.
